Permanent Supportive Housing

We are subcontracted by U.S. VETS to administer our Permanent Supportive Housing program to fourteen families on O'ahu. The program follows a Housing First model, an evidence-based client-driven program rooted in the philosophy that housing is a basic human right. Families are referred to the program through the Coordinated Entry System (CES) run by Partners in Care, Oahu's Continuum of Care. CES works to connect the highest need, most vulnerable persons in the community to available housing and supportive services equitably. The program combines long-term rental assistance with ongoing case management  and supportive services to help increase housing stability and improve the health outcomes for chronically homeless families.
